广东省环境保护厅关于揭阳市人民医院核技术利用改扩建项目环境影响报告表的批复

 

揭阳市人民医院:

你单位报批的《核技术利用建设项目环境影响报告表》(以下简称报告表,编号16FSHP021)、揭阳市环境保护局的初审意见和省环境辐射监测中心的评估意见收悉。经研究,批复如下:

一、你单位核技术利用改扩建项目位于揭阳市榕城区天福路107号。本次项目建设内容为:

(一)改建位于放射治疗综合楼首层的已退役的钴-60治疗机房,建设成为电子直线加速器机房,新增使用1台电子直线加速器(最大X射线能量为10兆伏,最大电子能量为22兆电子伏,属Ⅱ类射线装置)用于肿瘤放射治疗。

(二)在外科住院楼首层CT2室开展放射性碘-125粒子源植入治疗项目,将放射治疗综合楼五层东北角7号病房设置为专用病房。碘-125粒子源日等效最大操作量为1.11E+07贝可,属丙级非密封放射性物质工作场所。

(三)将放射治疗综合楼二层PET中心1间休息室改建为甲癌治疗专用病房,使用放射性碘-131开展甲癌治疗项目,日等效最大操作量为7.4E+08贝可,属乙级非密封放射性物质工作场所。

(四)在外科住院楼18层建设1间胰胆管造影(ERCP)手术室,使用1台胰胆管造影装置(属Ⅱ类射线装置)用于胰胆管造影手术中的放射诊疗。

二、广东省环境辐射监测中心组织专家对报告表进行了技术评审,出具的评估意见认为,报告表有关该项目建设可能造成的环境影响分析、预测和评价内容,以及提出的辐射安全防护措施合理可行,环境影响评价结论总体可信,我厅同意该项目建设。你单位应按照报告表内容组织实施。

三、项目应认真落实报告表提出的各项污染防治和辐射防护措施,并重点做好以下工作:

(一)健全辐射安全管理机构,完善辐射安全各项管理制度。辐射安全管理人员和辐射工作人员定期接受辐射安全与防护培训并持证上岗。

(二)严格按照《电离辐射防护与辐射源安全基本标准》                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                         GB188712002)、《医用电子加速器卫生防护标准》(GBZ126-2011)、《低能γ射线粒子源植入治疗的放射卫生防护与质量控制检测规范》、《临床核医学放射卫生防护标准》(GBZ120-2006)(GBZ178-2006)等标准要求建设各机房,落实各项辐射安全与防护措施,严格辐射工作场所的分区管理,工作场所须设立电离辐射警示标志,配备辐射防护用品。

(三)加强放射性物质的安全保卫工作,完善防盗设施与措施,确保放射性物质的安全。按照要求建立放射性同位素使用台账。

(四)严格按照《医用放射性废物的卫生防护管理》(GBZ133-2009)要求落实放射性“三废”处理措施。

(五)落实监测计划,配备辐射监测仪器。定期对周围环境和工作场所进行环境辐射监测并建立监测档案。工作人员佩戴个人剂量计,剂量计监测每季度进行1次,建立个人剂量档案。

(六)你单位核技术利用项目的剂量管理目标值:工作人员剂量控制值低于5毫希沃特/年,公众剂量控制值低于0.25毫希沃特/年。

 四、项目建设应严格执行配套建设的环境保护设施与主体工程同时设计、同时施工、同时投产使用的环境保护“三同时”制度。项目建成后,你单位应按规定的程序申请辐射安全许可。

 五、项目的日常环境保护监督管理工作由揭阳市环境保护局负责。
